It has certainly been a challenge to undertake this project during the Covid-19 pandemic, and I would like to extend my thanks to the CCG digital team, our colleagues at NECS, our CCG Clinical Leads as well as our two Pathfinder Clinical Leads Dr Becky Gladstone (South) and Dr Tom Martin (North).

There have also been many individual practices, GPs, Practice Managers, Nurse Practitioners, Practice Nurses and Secretarial staff who have also given their time, resources and feedback to develop the Derbyshire Pathfinder tool.

The prime aim of Derbyshire Pathfinder, is to design a useful toolkit for front-line Primary Care staff in Derbyshire, developed by the staff themselves, so please do keep the feedback and ideas coming through to our project inbox ddccg.derbyshirepathfinder@nhs.net
