DCHS Wound Clinic Service referral process from 15 May 2023

The Wound Clinic Service referrals can now be made via Derbyshire Pathfinder in SystmOne.  The Wound Clinic Service page can be found under the Community tab on the Pathfinder home screen.  It includes integrated wound clinic referral form, contact details, referral criteria, supporting letters to patients, and other service offerings to consider for a holistic approach to wound care (i.e. weight management, dermatology, etc.).

EMIS Practices will continue to refer to the centralised Wound Clinic Service Administrative Email – dchst.woundclinic@nhs.net – using the new Wound Clinic Service Referral Form.  To ensure standardisation this is the same form as the form in Derbyshire Pathfinder.
