Community Pharmacy Consultation Service (CPCS) Referral Form available in Derbyshire Pathfinder
NHS England and NHS Improvement held a Derbyshire wide GP referral to Community Pharmacy Consultation Service meeting on 11th November 2021 providing information, support and clarity in identifying community pharmacies offering the CPCS referral service.
The Community Pharmacy Services referral form is now available in Derbyshire Pathfinder for all SystmOne practices and can be found through Derbyshire Pathfinder homescreen:
