Changes to the Referral Method for TIA at Chesterfield Royal Hospital from Monday 21 March 2022

Posted on:

From Monday 21 March 2022 the Stroke Team are moving to Consultant Connect for the Transient Ischaemic Attack (TIA) service.  Monday-Friday between the hours of 9am-5pm referrals will be accepted via telephone.  In longer-term plans we hope to expand these hours with a multidisciplinary team covering the TIA service.

Initially the messaging portal on Consultant Connect will be used as a fall-back system and for out of hours. This will include set questions along with some free text boxes and ability to upload separate documents including medical history and current medication. We also understand that it may be easier to replicate our referral questions in to a template on your patient administration system and therefore we also welcome this document being uploaded to the messaging portal.

This new referral method will enable adherence against the standard of patients seen within 24 hours of referral, ensure patients are seen in the right place at the right time, and provide a one stop shop of face-to-face appointments with all diagnostics completed on one day.

We want to ensure that this process is as streamlined as possible and that it works for both the referrer as well as the Stroke Team, and of course the patient. We would appreciate your feedback on this when we go live; what works well and what doesn’t work quite as well so that we can learn and make further efficiencies. Any feedback can be sent via the email address crhft.tiastroke@nhs.net with the subject header as ‘feedback’ or ‘query’.

The link to Consultant Connect and referral into this pathway will be available from 21 March 2022 on Derbyshire Pathfinder via Derbyshire Pathfinder Home Screen > Stroke > TIA
